well I zoomed in on this pretty close and I don't see any difference in the digitalization. I isn't photo shop work and it hasn't been retouched. I can see this being real, some kind of strange medical condition maybe, the forskin is sorta a sure bet its real, really hard to fake forskin. If you look close where his fingers wrap around the cock if it was enlarged in any way the fingers would be large also and it clear the fingers are depressed into the flesh.
