I see thats on a Tropical disease research site so, contrary to my initial reaction it may well be a genuine pic.

I can't seem to find how it fits into the rest of the site though, if it's part of an article then that would explain things.

Yes, I too recall reading and seeing something similar some time back and am inclined to think it could be genuine, but on the other hand, only the photographer and the guy in the picture will know for sure.

Elephantiasis is caused by parasitic worms in the lymphatic system. Joseph Merrick (The Elephant Man) most likely had neurofibromatosis, a genetic neurological disorder, although that has been disputed.
