It's from a reality competition show called Bromans that he and Rhi won the first (and only) season of. The male contestants lived as gladiators and competed (non-lethally, obviously) against each other with stuff like wrestling and stuff. Rhi even helped make a mold of his (soft) cock and balls that revealed that while another guy may have had a bigger softie, Tom had the largest balls of any of the men. We actually got to see a very brief clip of his full frontal self in the first episode while he was celebrating Rhi winning a challenge while the men were naked (they had to "earn" their gladiator costumes). As for this specific clip, the men had to sleep among each other outside and, likewise, showered among each other outside.
Also this was during his rugby phase before he slimmed down so much so he still had some bulk to him.
