You're correct. In the case of adult circumcision, those who need or want a circumcision have a certain bias toward liking the new model. Therefore, the only way we would ever get a truly objective assessment is to find 5,000 intact adult men randomly and circumcise them. Ten years later ask them what they prefer. Anything else has selection bias.
This is kind of why its so hard to ever do any studies on this. You would probably never get this, most guys who get cut at an adult stage either have to or want to get cut.
