It is usually a darker ring around the upper-middle of the shaft that is a circumcision scar.

Actually, some guys have it at the base too. At least, according to what someone posted on another forum. There's another method where they cut the skin from the base and pull it all back. The actual skin from the foreskin is still there but it appears to look like a cut cock. The scar isn't as noticed, being at the base. Not sure if it's real, but eh... sounds interesting.

Anyway, circumsissions aren't always in the same spot, especially if the person had a long foreskin to begin with and where the place the ring (that holds the skin together to heal).

Doctors who don't have a clue as to what they're doing is the biggest reason the scar tissue is so far down.

That would be difficult to do. The surgeon would have to carefully reconnect superior arteries and maybe superior veins. Doable but difficult.
