STD clinic ASAP! The pictures are lousy, but the first one clearly shows a small ulcer, the second looks like a hard chancre. In my book, that is syphilis until proven otherwise. I don't know what the syphilis numbers are in the UK, but here in the states, it is a major problem in our young, gay male population, and our black female population. The ulcer is painless, resolves on its own after about 6 weeks, and invariably wakes up as secondary syphilis several weeks later. Rash, especially on the palms and soles. This disease kills, untreated can lead to irreversible brain/heart damage. PLEASE, it needs to be seen, tested before you give it to someone else. No BS telemedicine please, this can be serious. It isn't a runny nose or a sore throat.

I was tested for all STI and STD including syphillis which all came back negative.

There is something else on the skin possibly a ulcer and further down towards the base possibly a bit of eczema.

I’m making an appointment to see my GP and hopefully he will refer me to a urologist.

It looks less severe but seems to be covering a larger area. That might be part of the healing process, but I don't think I'd be satisfied with that answer. Have you seen a urologist about this? I don't think its an STD, but you mentions eczema and that seems like it could be the case.

Your first picture, in this last set, seems to show something else. It looks like HPV on your foreskin. HPV can manifest in weird ways. I would see other doctors if I were you, not all doctors are good at their job.

I have made an appointment with my GP for this Friday and hopefully he will refer me. It it’s nothing transmitted which is good but if it’s something like genital warts I believe these are transmitted?

The head looks almost like scar tissue but from what I remember before seeking treatment it wasn’t spread quite this far not did it hurt during a wank which is the worst!

Okay, very happy to hear that your syphilis test was negative. When it comes to STDs, I love being wrong. The bump on your foreskin is a wart, my guess would be HPV, regardless, it should be removed and biopsied. The glans lesion is fascinating. Assuming you were born in 1988, you are extremely young for erythroplasia of queyrat. (Goggle it, you don't want me telling you anything else about it). It should be biopsied, it doesn't look like eczema. Psoriasis is a possibility, but it doesn't really look like psoriasis. Regardless, you need a real medical opinion, first hand. If I'm not asking too much, I would appreciate a private reply once you are correctly diagnosed. - A Friend

Visited the GP today. He said it is Lichen Balantis on the glans and he doesn't know what the lesions on the shaft are. He gave me a different cream 'Fucidin H' which is hydrocortisone and topical anti biotic. He said apply it morning and night for 2 weeks. If there is no improvement he will refer me to a specialist for a biopsy.

Fairly good for the NHS in my opinion. I shared my reservations about using creams that are known to thin the skin, but he said internal medicines are not available for this sort of localised skin condition. 100% not transmitted as all tests confirm this.

I also said it is painful to wash as it feels like the glans is full of tiny cuts, but he said give the cream a go...

We have been using the same detergent for years but that could have been a cause. Same with body wash etc. - I'vebeen using natural stuff for over a decade!
Manufacturers change the formulas of their products from time to time. I wouldn't bet that the tide laundry detergent from 10 years ago is made exactly the same today.

Also, In a lot of the cases, it can be the colouring or fragrance that causes reactions.

Understood. Offered for general knowledge to others. One other thing about reactions to laundry detergents and soaps is that you would probably notice it on your ankles because of socks.

So if you do use soap on the glans of your cock, STOP. Try washing gently with warm water only. Try that for two or more weeks. See if that doesn't take care of it. The surface of the glans of your uncircumcised penis is similar to the inner surface of your eyelids. It is very sensitive and will react to soaps. (This advise doesn't apply to circumcised men, their glans have changed).

It doesn't matter what you have been doing for years. It is possible to develop a sensitivity or an allergy at any time.
The steroid creams that they are prescribing for you obviously take care of the immediate inflammation. However, you have a resurgent effect once you stop. That would lead one to believe that the problem is a not eczema. Try switching to washing with warm water for a few weeks and see if that helps.

I became conscious of all the harmful and toxic ingredients in personal care over a decade ago, when I had mild acne. I’m using a soap free pareben, SLS free wash specifically for eczema however I only wash my penis with water with foreskin retracted twice a day.

The doctor said there’s no cure for it (he said the same for acne which lead me on to researching cosmetic ingredients and watching my diet). If that’s true, what good would taking a biopsy do if indeed it is lichen?

Sent images etc. and the Dr doesn't know what it could be. They said I can go to a sexual health clinic. (not has sex for 6 months, always safe)

I would tell you to change doctors but in England that's probably not an option. Good luck. I doubt its anything serious, if you haven't have sex in six months and you are always safe.
I had an issue with yeast a few years ago. Might want to check out the symptoms of a yeast infection.

The other thing with those detergents, I would change to IVORY for about 3 months at minimum and see where that goes. I would also be very careful with any of the fabric treatments or just not use them this will include bed treatments and include hotels and other places you may stay over. The other thing to consider is clothing that is laundered via a dry cleaning process, DON'T WEAR THESE AT ALL...They are TOXIC to life.

Thinning of the skin, this is going to happen as you age. You have little or no control. What little use you do with that topical ointment or cream in 2 weeks you might notice at 75yo is debatable and at that advanced age you will have other more important things to consider.

I suspect that this is in fact lichen schlerosis. You need to get your Doctor to refer you for an appointment with a urologist and if the urologist is in any doubt at all you need to get the urologist to further refer you for an appointment with the Andrology department at UCH in London - preferably with Mr Maneer.

I've been applying the cream morning and night for two full weeks and for the most part the problem has gone, at least on the head.

I have a couple of patches on the shaft which look like dry skin (size eczema) that are nothing away.

At this point I'm not too concerned, however the real test will be in month or so post applying the cream - last time I stopped using the cream is when it came back on the head.

Still considering using coconut oil down there which has had a lot of anecdotal evidence for these sort of skin problems but again, wait a month or so and see if it comes back.

Finding comfortable breathable underwear is still a challenge!

I agree with Hairy that the lesions on the foreskin are classic genital warts. Those should be removed by freezing, acid or prescription cream to prevent spread and transmission. The lesions at the base of the shaft could be warts as well. The pictures are slightly blurry.

To me, it look like eczema as I have similar patches on my leg and looking up genital warts on google images, I don't think that's what I have. There is also a similar skin patch on my sack.

And its back again :(

I've been applying the cream for 2 weeks and since stopping last week it looks to be ineffective.

Doctor said if this happens he will refer me to a dermatologist who will take a biopsy :worried:

If this is Lichen Balantis, from what I understand there is no cure, only measure to manage it such as applying creams.

I have a long history of digestive disorders (like IBS) but from talking to my Doctor, it's unlikely the systematic factors will be looked into - it's "just a skin condition"
