The more photos you show, the more I am convinced (from experience) that this is lichen schlerosis. Without wishing to alarm you the affected skin around the base of the glans will eventually fuse itself to the glans and then cause painful tears during sexual activity. You should use Daktocort cream to hold this at bay as long as possible. As I said upthread your route through the NHS needs to be to get your Doctor to refer you to a urologist and the urologist to refer you to Mr Asif Muneer at the Andrology Department at University College Hospital in London.

I'm of the sense that my Doctor this I'm paranoid based on asking more questions that most probably do and getting prints out of my test results... My question is, how would I say to him I want to see Mr Asif Muneer some 300 miles away from me, if my doctor know there are other local options? I'm assuming there are many doctors like Muneer???

Your Doc may think you are paranoid which is why you need to escalate to a urologist, who won't think you are paranoid. There may be andrology departments closer to you and these may be within your NHS Trust Area. AFAIK if it is outside your NHS Trust Area you can go anywhere and Mr Muneer is worth the train fare. There is no real treatment for lichen schlerosis. However should you end up with circumcoronal adhesions then you will in all likelihood need surgery. Google "rolocs procedure".

I'm calling for an appointment tomorrow, should hopefully be referred fairly soon but I have no idea on waiting times, could be months.

Did you or do you still have LS? What did Mr Muneer advise you?

Some research suggests circumcision... Whilst I'm waiting to see a specialist would retracting the foreskin 24/7 be worth a shot? There must be some sort of device to do that without injury.

I'm around 8 years since first appearance of LS. Still have active LS. Have the adhesions and due surgery within weeks. In the meantime can only advise you use Daktocort or you may be prescribed a topical steroid cream if/when a urologist has diagnosed..

8 years is a very long time, but at the same point many live with conditions such as excess their entire lives. Has is effected you sexually? Are you having a circumcision?

Sent images etc. and the Dr doesn't know what it could be. They said I can go to a sexual health clinic. (not has sex for 6 months, always safe)
The trouble with an online dr is that they only have snap shots of what it looks like and your descriptions of what it feels like. Personally I would just leave it alone, if it doesn’t bother you, our bodies change over time it could just be one of those things.

It’s clearly concerns you however so I would heed others advice and seek out a dermatologist

LS is a condition where the symptoms can be kept in check somewhat with a topical steroid cream. Symptoms will come and go as you yourself have experienced with the use of creams over the past few weeks. If your condition is confirmed as LS then you will in all likelihood experience flare ups of the symptoms permanently. This could eventually cause scarring of the skin which may require surgery, either a traditional circumcision, the rolocs procedure you can google or a cuff of affected skin excised. Again, GP -> Urologist -> Andrologist is the way to go.

My favorite is Palmers Coco Butter. I do a have jar of pure coconut oil on my vanity but I use that only sparingly.

I have used baby oil for years before I switched to Palmers. I never had any problems and it did keep the skin on my cock and balls soft. I started using baby oil right after I started keeping my shaft and balls cleanly shaven. Still keep them shaved and my pubes trimmed. I m always conscious about good hygiene.

After using the cream for 6 days, the issue has completely gone

It’s likely to be contact dermatitis and having changes out most of my underwear this past month that could have contributed to it. I’m going to stick to cotton and bamboo underwear for now as this allow better ventilation etc.
Under armour underwear is great for ventilation! Not the cheapest but you get what you pay for

So the issue has come back. Worse

The cream 100% cleared the problem and I stopped using it as prescribed. The past 2 weeks the same patch on my glans has become slightly itchy and sometimes burns during masturbation.

I'm working on replacing all my underwear with bamboo but also going to the Doctors again. I'm reluctant to use the cream because prolonged use can cause thinning of the skin which I definitely don't want!
Sounds like a fungle infection that didn’t quite go the first time around. I understand you don’t want to thin the skin but do you want to continue with this infection?

Also any time I’ve had a fungle infection (on me feet and arm pits) very regular washing and thorough drying along with as much airing as I could manage. I know it’s not ideal to air your glands but a thought...?

STD clinic ASAP! The pictures are lousy, but the first one clearly shows a small ulcer, the second looks like a hard chancre. In my book, that is syphilis until proven otherwise. I don't know what the syphilis numbers are in the UK, but here in the states, it is a major problem in our young, gay male population, and our black female population. The ulcer is painless, resolves on its own after about 6 weeks, and invariably wakes up as secondary syphilis several weeks later. Rash, especially on the palms and soles. This disease kills, untreated can lead to irreversible brain/heart damage. PLEASE, it needs to be seen, tested before you give it to someone else. No BS telemedicine please, this can be serious. It isn't a runny nose or a sore throat.
Did you not read the thread? He’s already been and been cleared...

The cream I was prescribed also contains a antifungal (or to that effect) but it didn't really help. Certainly no more than a steroidal cream I was prescribed first time around after my STI/STD tests.

Just came back from the Doctors appointment.

He had another look at it and gave me a different cream. Betonvate which is:

Each 1 g contains 1 mg of betamethasone (0.1% w/w) as valerate. The other ingredients are chlorocresol, cetomacrogol 1000, cetostearyl alcohol, white soft paraffin, liquid paraffin, sodium acid phosphate, phosphoric acid, sodium hydroxide and purified water.

He said it could be an auto immune issue and asked me if I'm anxious or depressed. I sort of said yes to both. Then he said natural treatments of the body are best, which honestly surprised me because in the past he always pushed drugs for acne etc.
