No one considers elective surgery, unless he wants to improve something about himself. Some achieve this goal by joining a gym, and, working out. Others do this by coloring their hair. This goes a step beyond that.

Yeah.. but... The 10" person who wants more does probably need to look at what EXACTLY is driving their particular needs for more. Is it purely to get a few more porn gigs, and will it pay for itself and be worth the risks? Or are they perhaps laboring under an unhealthy obsession?

I know a few guys who have perfectly wonderful bodies but whose body dysmorphia has led them to disfiguring, painful, and in a two cases life-threatening modifications. They needed psychologists, not a plastic surgeons. So it's worth exploring because a huge guy who takes extreme measures to be even bigger -- that should ring a few alarm bells.

I’m more than happy to settle for a guy with 4 to 5 inches. There’s more to a man than the size of his cock. Like his charm, personality, good looks/body, his broad shoulders, big hands and most of all, the size of his wallet!

For what it's worth, 4-5 inches is only a little bit below global average size. And I'm unaware of any surgery that can change length by anything more than an inch or so. Those surgeries don't actually create length, they just detach connections allowing existing structures to stick out a bit further -- resulting in a penis that gets hard but not "erect" in terms of pointing towards the ground rather than outward or upward. You can see this a lot in porn, where the guy is hard but has to manually aim his dick because it's no longer got any "direction."

It doesn't actually increase erect length. The only thing it really does is make you look more impressive in the locker room by making your flaccid dick hang down a bit further.

You don't have this surgery for sexual performance, as it just makes that worse, you have it for other reasons. This is the most important thing your friend needs to educate themselves on. What exactly they are thinking they'll get from the surgery and what it is they are actually going to end up with.

What would be your reaction?

I'd strongly advise against it. Surgical PE consistently sounds like bad news to me. Further, I'd probably recommend that he start seeing a mental health counselor. Being so driven to change one's body like that is usually rooted in irrational and maladaptive insecurities.
