My erections vary, and sometimes I have one similar to what you've described above. There are so many variables that it's hard to tell what's going on, but here's a few things that I've learned:
1) There are two components to an erection: a) physical stimulation and b) mental stimulation. It is my understanding that if you play with your dick without really being sexually aroused, you will acheive an erection of the shaft, but the glans (head) will not be all that hard. I mention this because if you've got a lot of stuff going on in your life and you are distracted, you may have the semi-hardon that you describe.
2) MANY, medications fuck with arousal. I take Propecia for hair loss, and that can cause erectile disfunction (keep in mind that disfunction doesn't necessarily mean a complete loss of erection, but can mean a partial loss as well). SSRI-type anti depressants are notorious for this. If you are on meds that you can safely take a break from, ask your doctor if you can come off them, and how long you should wait for the meds to clear out of your bloodstream and then see if the problem goes away.
3) If you haven't had a complete physical in a while, not a bad idea to go ahead and do that. Tell your doc what's going on and see what he/she has to say. PLEASE don't waste time being embarassed. Doctors are sexual beings too, and they understand the importance of sex in the life of human beings. If your doctor doesn't take it seriously find another. Remember, if this is on your mind, it should be on theirs.
