The frenular region contains thousands of meissner corpuscles, which are the same class of nerve endings found on the lips, palms, and soles of feet. In the penis, stimulation of these endings contributes towards building up orgasm (i.e. they are a trigger of orgasm).

Due to the physical structure of these nerve endings, they can be innervated in a couple ways. This is because all that is needed to get them to fire is a small deformation of their shape (these things are very tiny and right at the surface of the skin).

One is direct stimulation, which deforms them, and another is stretching. Some intact males can reach orgasm simply by retracting their foreskins - the stretch alone can slowly build up to orgasm.

If you watch carefully, this guy retracts his foreskin, and while he doesn't retract it fully and stretch it tight, there may have been some passive stretch due to the tension of the peripenic muscle tissue (a ring of muscle which tightens the skin).

This is just one guess at what was going on. It could be hidden prostate stimulation, or he may simply have discovered or been gifted with a way to activate those pathways from higher cognitive centres. Or it could be a combination of any of these possibilities.

My money's on the stretch hypothesis though.
