What's that thing in the shower?

I want to get one of those cool shower head atachments, But i am worried that my houseguests will ask about it. I could use my favorite line from the movie Parenthood, "It's an ear cleaner" All my guest know I am gay, But they might feel unfomfortable showering with my ass cleaner. Do the come off and have something to cap it off with?

Not to mention the one I saw is like 90 bucks. Not cheap, But it seems more comfortable than douching. Any thoughts or recomendations.
