Cute post FuzzyKen but i am sure some guys do have their dicks altered. If your dick is mauled up looking then it is probably because of some type of surgery that went wrong. To do lengthening, an incision will normally be present inside the pubic hair. There are several different scar configurations. When the surgeon goes in, it is for the purpose of cleaning out, altering and total removal of ligament structures. Most MD's doing this even now use either an "M" or a "W" shaped incision. This is done because the MD needs to make an incision he can stretch without tearing the tissue.
These are called "M-plasty" and "W-plasty" respectively. There is a new technique under development that goes in beind the balls on the frenum but that has not been released yet and I can state this for fact because I know the two surgeons developing it.
Girth enhancement surgery places incisions right behind where the circumcision scar line would be. A surgeon doing this would then insert either the dermal graft or the alloderm based prosthesis in under the fascia of the penis.
A real enlargement surgery usually costs well over $10,000 with what is called a dual augmentation procedure. This is usually a wee bit outside the budget of most people in porn.
What I can attest to these days is the number of men who have ventured across the border to Mixico for the illegal injection of medical grade silicone. In some men what happens over time is that this forms nodules which have to be surgically removed. A penis that looks like it has been through a war was most likely the result of Mexican Silicone injections which is far different than PE surgery.

Tbookx and Fuzzyken got it right on the lengthening surgery, I read a good article about these procedures. Apparently, any guy has an inch or two of penis he can't use, and severing the ligament that holds it up against the belly subsequently lengthens the dick one or two inches. From what I've read, its very routine and has no effect on erectile function. I'm pretty sure Mandingo and Lex have had it...sometimes you can see the scar tissue on their abdomen right above their dicks. Its really easy to see if their pubes are shaved.
