I'm pretty sure the guy on the right is Karel Grosse; he goes by another name, now...Jake (?)

100% it's Karel. There are dozens of vids and pix of him around.

It's one of his earlier vids, 'Wank Party' by William Higgins made around 2006. He seems to have settled into the name of Trevor Yates now but has been victim to that eastern European habit of renaming performers for almost every vid. You'll also find him as Michael Long and Pavel Kreb.
