As for the oringinal question, there's really no good reason for most women to hold off an orgasm unless they're just trying to do an experiment. Women are usually able to continue to have sex and continue having orgasms almost indefinitely, so why hold off a good thing?

Digger, I've been there with guys who have that come up from time to time. It's really no big deal, I think it probably happens to every guy at some point or another. If it happens, just smile, relax, do some oral on your lady until you're ready to try again.
If you don't make a big deal out of it, it won't be a big deal and you'll probably find you'll be back to your old self soon enough. The worst thing you can do is get all frustrated and kill the mood while she's still aroused. If you don't do that, it will work itself out.
