It makes zero sense for even a single one of them to be real. Logistically it would be stupid to do it any other way... If you are going to already make several prosthetics, you're just going to make them all. You won't have to juggle which days it's legal or not for someone to be nude on set (while maintaining records of such). And since every single naked body was a bloody, grime covered Zombie, you aren't going to have makeup artists applying and re-applying makeup to real human genitals.

They also shot this movie and it's sequel back to back with a 3rd in the works. Those fake dicks are practically Wardrobe at that point

Unfortuately they're all fake, Danny Boyle confirmed they couldn't use real nudity because of the laws around children being there during filming.
