There’s different tipping cultures in different countries. However, I’ve found in Europe that if your massage therapist gives you an extra that either wasn’t requested up front or was more than was expected then it’s only polite to tip 10 or 20.
In some establishments the staff will be on a daily rate and so any extras should be paid direct to the staff through a tip.
If you’ve already paid for a ‘special’ massage at the start then don’t feel obliged to tip unless you really thought the ‘service’ was worthy of extra praise.
Again, it really depends on where you are and the therapist . I’ve had normal massages that have gone sensual and one therapist made me aware that it was going to cost extra. Another therapist at the same place didn’t and when I asked her she said there was no need to pay any more.
