He injects his scrotum with a IV type bag of NACL *actually a bag and a half* They give this same stuff to you at the hospital to help keep you from dehydrating. It stretches and swells the scrotum and sometimes penis giving them a bloated heavy hang. It's temporary and safe, as far as the solution to body goes...the only issues are doing too much OR not keeping injection sites sterile and getting an infection. Done right...it's pretty safe. It' lasts for a few days...depending on how much you do. *it's fun too!*
