I had surgery 12 years ago for a varicocele on my left side at age 23, and since then, I've been unable to wear microfiber underwear (the same thing happened to me before) because my operated testicle starts to hurt more and more. If I wear it for more than a day, it starts to grab my butt and my entire left leg, becoming unbearable over time, and I have to take it off. If I replace it with cotton clothing, magically, the pain disappears, and I don't have any discomfort.
A year after the surgery, I went to the urologist. He told him about it. He ordered another scrotal ultrasound to make sure I didn't have any more varicoceles, and he said nothing. He didn't give me an explanation for why this happens.
Nowadays, sometimes I feel pressure on my perineum, stuck to the scrotum. Sometimes it bothers me more, sometimes less, especially if I'm up for long hours without sleeping.
Is there any scientific explanation for this? I'm wearing microfiber and cotton? The only explanation I can find is that cotton dissipates heat and microfiber retains it, but there must be something to it, because it's causing this for me, since the doctors didn't give me any importance when the ultrasound was negative.
I also have some varicose veins in my left leg.

I appreciate any ideas of what it could be. Thank you.

Why not wear 100% cotton boxer briefs with a supportive pouch to help keep your balls cool and as far from your body as possible?

Besides, most brands offer that kind of boxer, so it’s pretty easy to try a few and see what works best for you.

That said… I might be willing to sell you some used ones, if you’re into that.
 
Yes, of course, I only use cotton.

But my question is focused on finding a scientific medical explanation, that is, knowing why this happens. Obviously, I don't use any microfiber.

Microfiber or synthetic underwear can be harmful for varicocele because it traps heat and reduces breathability, increasing scrotal temperature and moisture.

This can worsen symptoms and cause discomfort. For varicocele, breathable, natural fabrics like 100% cotton and supportive underwear (like boxer briefs with a pouch) are usually better.

At the end of the day, a varicocele are like varicose veins on legs, so optimal support or gentle compression is recommended to alleviate discomfort.

It’s possible that microfiber increases heat and compression, and worsens nerve sensitivity or vascular congestion, especially if you have residual vein issues post-surgery. Cotton's breathability and reduced friction could help minimize pressure and temperature buildup.
