ED issues (again)

I know there are a ton of these threads, but I haven't really seen anything for my particular situation. Besides, all the threads I've read bring a ton of "yeah me too" posts so maybe it's good to discuss more regularly.

Anyway, over the years the quality of my erections had degraded to the point where I've lost an inch or so on length. Cialis worked great for a while, but lately not as well even at max dose. The shaft of my cock gets rock hard with it, but the head never really fills and the root (in the taint area) is wobbly, so that I'll slip out or buckle.

I loved edging when younger, and I do wonder if that did damage to nerve/blood vessels.

Considering asking about injections, but obviously not something I really want to do and not sure it addresses the particular issues I have.

What's weird is from time to time, I'll still get some that are high school quality. That'll usually last a week or so.

Here's the other relevant details (gleaned from other threads):

Overweight but not deathly so, about 28% bodyfat (and working on it). Lift 3x week and long walks each night (but a desk job during the day).
Testosterone is 650, measured recently. No therapy. Estriadol was high (57) which is weird, but that test wasn't as accurate.
I can still orgasm regularly, sometimes multiple times per day with chub or half mast. It isn't as enjoyable or powerful as when rock hard, however.
No PE.
Considered a cock ring but since the issue isn't really getting the shaft hard not sure it is appropriate.
Still have strong sexual urges.
BP is 114/80, bloodwork is normal across the board.

See a urologist or a dr who specializes in sexual health. It might be a slight blockage in the penis. They can detect one pretty easily. May need one of these tests:

1. Penile Doppler ultrasonography​

  • It helps doctors see if there are any problems with blood flow to the penis, such as reduced blood supply.
