it's expensive as hell. not all countries with universal healthcare cover dental, and even so, something like invisalign would likely not be covered, regular braces only, which is a hassle people won't wanna deal with unless they really have to.
on top of that, our idea of dental health is very different from the american ideal. tooth whitening isn't really a thing done unless it's influencers and celebrities, and even a lot of them either don't do it or do it to a much lesser degree to just remove general yellowing, not to have pearly whites. we don't get veneers, because we think they look weird, fake teeth are implants for extreme cases, not aesthetics. our wisdom teeth are removed with local anesthesia and ONLY if they're showing signs of problems, such as already having cavities before they're fully out through the gum, or being born sideways, which will cause lots of pain. getting our teeth straightened is usually more for extreme cases for most people. situations where it's considered a need not a want
