It's possible, he's wearing jockstrap underneath, you can see the straps.
Either he puffed up, or he used his underwear to disguise what he already has and didn't even need to puff up...

I'm unconvinced. It makes more sense that the jocktrap was worn so he could wear a cup. If he was that big, I think more people would know about it - he'd already have a thread on here. Are there other pictures of his bulge about?
