It's extremely easy to swap faces. Heck, Anyone can make convincing deep fakes these days. But, they could have also swapped the underwear with imagery from the body double.
That's never been proven to actually be him and I really don't think it is. That seems like some drunken frat party which he probably wouldn't have been a part of at peak fame. We don't even see the guy's whole face.
