Penis injury during sex

I am a top gay man. I am on prep. But my issue is that every time I have sex I have injuries on my skin like small skin burn from traction I guess. Every time i use lube but still the same. Even with a condom again the same problem.

Please give me sime advice what shoukd I do to prevent my dick getting this. I noticed that in recent yeat and a half this started to happen.
