Yes it is real milk.
Men do have the potential to lactate but only in special cases. One of which is when you have high estrogen in the body. See if one takes a lot of excess exogenous testosterone like anabolic steroids, the body's testosterone is converted to estrogen by the enzyme aromatase. Estrogen then promotes breast tissue development and along with prolactin, which is not only already present in males but becomes higher then synergises with estrogen, promoting milk production. Hence why his titties are also so big.
