Last week I was diagnosed with stage 3 Prostate Cancer at the age of 42. I am blessed that it has not spread anywhere else, but I will have to have surgery to remove my prostate. Has anyone on here gone through that who is able to share their experiences with me? Drs. did inform me that after the prostate removal, I will not be able to ejaculate, it will be dry. I would like to get opinion of those who have gone through this as well. Any thoughts or experiences are welcome. Thank you in advance.

I had a positive biopsy at age 72. Had minimally invasive prostate surgery within a month of diagnosis. Surgery was uneventful as was recovery. Doc was optimistic he got it all and for four years I had clean exams. At the 5th year follow up, there was positive PSA . Then followed up with radiation Have had some positive psa at extrenely low levels on an ultrasensitive test. Doc is not cocncerned about it as long as it is close to zero.
Sexually, it was a roller coaster. Could not get erections after surgery doc recommended pills, pumps ,etc. got some marginal benefit from those but it was unpredictable. Doc then recomended injections. I used trimix for a year or so, still with inconsistent results. Finally he said your next and last stop is an implant. My reaction was they are not doing that to me.. After a particularly frustrating excapade, wife and i agreed to get the implant. I am the proud owner of a coloplast Titan, 23cm. It works like a charm consistent erections, no hassle with pumps and pills. Wife and I had discussed jsut toughting it out with "toys and tongue", but the desire to return to normal sex won out.
My pre op advice would be to get the best surgeon you can. I am in Houston and used Dr. Mohit khera with Baylor/ Other Houston docs are Dr. Clavell and Dr. Starke with Methodist.

Is it true that you are not able to cum after the surgery? How does that work for you? Was it hard for you to accept that?
I am in Houston as well and seeing Dr. Chery at MD Anderson. He has great references.

Is it true that you are not able to cum after the surgery? How does that work for you? Was it hard for you to accept that?
I am in Houston as well and seeing Dr. Chery at MD Anderson. He has great references.
Bighornso: You will enjoy and not regret the implant. Regarding 20s,sex, it doe not turn the clock back to your 20s. The sex is great once you heal. It is different however. If you are having protatectomy you will climax, you cannot ejaculate as the plumbing for that is removed. I still produce pre cum as that is made by the cowpers gland that normally is not involved with the surgery.
Advantages of an implant are you stay erect until you deflate. Sex is not over 2wo minutes after the nman climaxes. Others are what my wife calls "dial a willy. She likes it medium firm for piv sex, fully inflated for shallow oral and somewhat flexible for deep oral. When will also as me to inflate or deflate depending on mood, position, etc. Again, be glad to talk, and/or show and tell.
I had no trauma around my changed status. I was very fortunate, my wife was with me 1000% and knew what was going on. We discussed eveything and she was very supportive.

Thanks for the information. I think I am just bummed about the fact that I will not be able to ejaculate. It is just something I will have to come to terms with it.

Is the implant expensive? Painful? Do you have pictures of it to share?

Big hornso: You need to move beyond inability to ejaculate. You can still climax and depending on how many nerves the doc can spare you may easily grow back to regular sex. The implant was essentially painless. Dr. Khera used a peno scrotal incision which was under the penis on the scrotum about an inch below where the two join. In hospital overnight, it is expensive. Abour $20,000. Medicare and hosp stook care of about 90% of the cost. I have to look back thru albums and see if I have any immediately post op photos...You will have to struggle though the other less invasive ED fixes before thy will approve Medicare and hospitalization. Once healed, the incision is not even identifiable on my sac. I don;t think those will show much. What is more informative is a show and tell.
Other choice of incision is infrapubic, on the abdomen about 2 inches off center from the penis. i tell guys to let the doc used the incision they think is best considering anatomy , prior surgeries, etc.

Thank you, I am adjusting to the idea of not been able to ejaculate. I think it's all new to me so it will take some time, but eventually I will make mends with it.

Hi,I'm a prostatectomy survivor also. I took was able to catch my cancer early at 58,and I will be 73 this year. Be very grateful,I have lost friends around your age to this cancer. As others have replied, getting an inflatable implant installed is the BEST,just be sure you find an experienced doctor. I had the robotic nerve sparing procedure too. I would suggest going to franktalk.org ,there you can create an account,it's free. Lots of articles from survivors, implants Ed meds, whatever. You can DM me on my profile,and we can discuss this further. Be positive,ask a lot of questions,be your own advocate and include your wife/ partner. You will get through this,sex isn't over yet.
