There's not really that many things you can do. The skin can be filled with some sorta filler, for example thermal fillers which are synthetic or your own fat. There's also a ligament cut surgery which sometimes makes it look longer when soft. Then there's that synthetic sleeve thing that goes under the skin.

All the things that can be done ATM, make the dick look absolutely disgusting and unnatural. Never seen a nice result.
