I’m sure many disagree, but I don’t think this is a healthy fetish. I know a lot of people write it off as “it’s just fantasy man lighten up!” but it seems darker and more sinister than that to me, and I think (almost always) stems from a deeper sense of self loathing. I don’t partake in any aspect of this, but it comes across as severely mentally and emotionally toxic.

I disagree. In my opinion, as long as all parties involved give their informed consent and keep that sort of dynamic only to the bedroom it's a valid way for small guys to try and deal with their indequacy by regaining a level control over how they're perceived sexually in a society that practically worships big dicks. Now, I don't think there's anything wrong with big dick worship; I partake in it myself because big dicks are fucking awesome but you need to keep in mind these guys want to be belittled, humiliated and put in their place. At the end of the day, it's basically just a variation of your more typical dom/sub sexual dynamic.
Even still, not a healthy attitude towards yourself. Do short men like to be humiliated because society praises tall men? Do ugly men enjoy it because society praises handsome men? Sex and kinks/fetishes are often used as a front for bizarre and harmful behavior that would otherwise be deemed unacceptable. And I firmly believe it leaks over into real life and isn’t nearly as separate as many would like to convince themselves.
