He's a rapidly declining mess, all right, and a perpetual self-reinventor; and those ridiculous, ugly tattoos and ridiculous, ugly nails are real enough. But, apart from being in one of his slightly skinnier, less muscular phases (he's gone up and down before), not much else has changed, outside of the specific staging of his photoshoot. Same haircut IRL, same mannerisms and personality. He's smoked cigarettes, rather heavily, the whole time, at least since he was wrestling at Columbia (and therefore probably before). He certainly didn't pick up a drug habit or anything in the military; that kind of thing isn't done anymore. He's probably just going through a post-military-washout "self-expression phase," which actually seems rather mild in the perspective of those. And, of course, found a new way to hold his public's interest for just a little more.

Bottom line, most of what you see here is just specific to the photoshoot. And people take the fantasy stories told by those way too seriously. (Most people you see smoking in them nowadays aren't even smokers, for example! And few subjects are as camp--in any way in question--as they're made up to be by the photographers.)
