Hello! I'm turning 23 this year, and for the past 2 years I have been struggling with an ED issue ( I think) that was caused by me which I will absolutely regard as the stupidest decision in my whole life, until I die.

I'm planning to go to another urologist this week to hear what they have to say. Any help or suggestions from you guys would be very appreciated. Idk where else to go.

I'm also planning to buy some pygeum ,saw palmetto, and l arginine supplements but since such things are quite expensive (I have the money, I just don't want to waste it), can you share your stories of your experience with said supplements, possible side effects (this one I'm really afraid since I am traumatized by some meds a psychiatrist gave me), and overall some advice on usage and good brands? Thanks a lot!

I think I'd be feeling the same as you, especially if I felt my urologist was dismissive over my symptoms. For any man, it can be very distressing to have this sort of experience but it's especially tough for guys your age.

I do think it is unlikely that you've damaged your prostate. If you had a prostate exam then you'll know first hand how difficult it is to actually get to it so I think it is unlikely you've done something to your prostate by pressing on your taint. The perineal muscles do influence our sexual function as well as bladder control so if everything else is OK, like you're not having any other symptoms like bleeding or problems urinating, I think it is less likely that you've damaged your muscles.

When I worked as a hypnotherapist, I would occasionally have men, sometimes quite young men, come to me who have had issues with getting or maintaining an erection so I have seen first hand how distressing this is for guys. The idea that this is "just" a psychological issue can feel really dismissive but the truth is that mind can have a really powerful impact over matter. If we're worried about not getting an erection or not having a good enough time or are anticipating problems then it is likely that this would have its own impact which is something I worked on with the guys who came to see me. When they were able to stop worrying about things "working" they were able to actually do what they wanted to do.

If you're thinking about supplements, I found that pygeum was the only one that had any noticeable effect and it seemed to make me precum a lot more than usual. Please make sure that, if you do take supplements, you get them from respectable suppliers but also take into account that the supplements you talked about would do nothing to affect a perineal injury. If you do masturbate less, or practice edging where you deny yourself that climax, you will eventually find yourself having more semen and being more stimulated.

If you've had a traumatic experience with medication from a psychiatrist, it is worth noting that if you are currently taking any medication for your mental health, sometimes side effects can be problems with sexual functions. See a different urologist. Get an ultrasound. I don't think it's your prostate but have your whole anatomy checked. Damage to the vas deferens?

Disclosure: I AM NOT A DOCTOR, I AM SOME GUY WHO SENT YOU A SUGGESTION ON A PORN/PERVERSION SITE. But still. If you can go to a university research hospital that's the ideal choice. Expertise. Get checked out. Tell them specifically that you are concerned by the last doctor's dismissal, and by the fact that by the sound of it some time elapsed between when this started happening and when you were first able to seek medical attention.

As for a regular doctor in your new destination, get an internist, not a general practitioner. They have a lot more training, know all the body's systems well and can coordinate with whatever specialists you need. If you can, find a sex-positive clinic and practitioner. You might try any clinic that specifically calls out that they have a practice that's LGBTQ+ friendly. Those doctors won't blink at whatever comes through the door. They will have seen much stranger things than you, and won't judge, you can speak freely. They could refer you to the urologist.

You should definitely see another doctor. I don’t think it’s something to do with prostate. I agree with @neintynein

when it comes to supplement, I used Now Foods: Pegyum saw palmento supplement and it was giving me a lot of precum but again I don’t believe it’s related to prostate. Please see a doctor and try not to turn this into a psychological trauma

Update: I went for online teleconsultations (because my mom is visiting) with 2 more urologists and they gave me the same answer: live healthily, workout, eat well, et cetera. So again what they have concluded is that it's more likely to be a psychological problem. I'm still not 100% buying this whole mind crap thing but I guess it won't hurt to try doing their advice. So I went and bought some foods like dark chocolate, nuts, coconut water, anything the internet say could have the potential to boost my performance.

If you guys have any suggestions or recommendations on what food, snacks that have proved to be effective, that would be awesome!

P.S one of the doctors mentioned the possibility of me experiencing what seems to be varicocele. It's like the blood vessels inside your balls are swollen or something and it might be affecting my penis.

Thanks for the insight, I haven't even heard of retrogade ejaculations before! But the "cumming but no cum" thing just happened the first time I tinkered with my perineum. All subsequent ejaculations, even to this day, has me shooting cum (albeit it does have a more runny consistency and not that thick nor white compared to my cum 2 years ago)

What brand did you use if I may ask? Word of mouth is always a priority to me.

To understand what may be going on here, I think it would be useful to see some pictures of the relevant anatomy. This picture shows this from a urinary perspective, i.e. it omits the reproductive bits, but is a good starting point:

The things to here are the location of the prostate gland and the pair of sphincters both immediately upstream and downstream of it. The upstream one, just at the point where the bladder neck becomes the urethra, and called the internal urethral sphincter, is involuntary, i.e. you cannot consciously control it. It is normally closed but opens automatically when you need to pee.

The downstream one, called the external urethral sphincter can be conciously controlled. It is this you use if you need to pee but want to delay it because you aren't somewhere suitable.

Now to add in the reproductive function, look at this:

Sperm make their way from the end of the epididymis up the vas defrens. As far as I know that isn't limited to ejaculation but happens all the time, which is why you need time after a vasectomy to be clear of sperm because, at the time of the op, some are already up inside the body ready for ejaculation.

When we ejaculate, the sperm are joined by fluid from the seminal vesicles, then that mixture is joined by yet more fluid from the prostate gland. All of this ends up in the bit of the urethra that is inside the prostate gland, between the two sphincters I already mentioned. Assembling the semen there is called the emission phase of ejaculation. You can actually feel that happening before anything spurts out and it feels good.

At that point the internal sphincter will be closed*, to direct the semen outwards and not into the bladder, and the external one will be open to allow it out. Finally muscular contractions push the semen down the urethra and out of the penis.

Pressing on your taint while cumming will have had the effect of blocking the urethra downstream of the external sphincter, much as you can cut off the flow in a garden hose by pinching it. The muscular contractions still happen to a piece of the urethra now closed at both ends: by the internal sphincter at the upstream end and the blockage from the pressure on your taint at the downstream end, so the pressure inside will increase. Something has got to give and what usually happens is that the internal sphincter leaks and the semen ends up in the bladder. That is the retrograde ejaculation referred to earlier.

What is not clear is why that happening as a one off should cause things to malfunction thereafter. Because the internal sphincter is involuntary, I can't see how you could strain the muscle involved. I know some people do this thing of pressing on the taint regularly and seem to get away with it, others warn against it. It does seem hard to believe that the whole load would go upstream through a slightly damaged or malfunctioning internal sphincter when the downstream route is clear, unless that sphincter is wide open.

As for a physical cause, there is no point in considering anything upstream of the seminal vesicles as those and the prostate gland make up most of the volume.

I'd say it was worth seeing a Urologist who was prepared to do a physical examination. Even if he finds nothing amiss, it's worth it just to be reassured. At your age things should still work reasonably well, although there will come a time when things start to head south and performance becomes unreliable. Are you still weeing okay? If urological function is normal, I'd inclined to not be too worried.

I am still urinating as usual. No pain or anything. Ejaculation still works and I usually shoot out a lot provided I do some edging beforehand.. it's just that the orgasm and erection have both decreased in quality..

Sometimes I experience a stinging pain near the scrotum - perineum part though. But it happens like only 7% and very randomly without any cause whatsoever.

Thank you, I very much appreciate your detailed analysis on this. I don't think it's a case of retrograde ejaculation though (maybe the first time , it was) since to this day I am still cumming normally...

At this point I still don't know what's happening to me, but I'm trying to enhance erection, orgasm, and produce quality by doing kegels, eating aphrodisiac foods and taking my supps. Hope it'll work!

I'm taking multivitamins as well as celery seed (as I've heard that it increases precum production) and if I'm not mistaken it gives the same effects as stinging nettle root based on what I read online about it
