My advice is that she gets a professional fitting hunni, not a mainstream store but she looks for a small independent store that specialises in large busted women. I have to travel a fair distance for my fittings to a little store but it's totally worth it, the owner knows everything there is to know about large boobies and what's best for them.
If she's suffering from back pain she's totally wearing the wrong bras, a good properly fitted bra should support from the underneath and put most of the weight into the band and not on the shoulders, that's why she'll be struggling.
