for me, on the day going commando, just unzip/unbutton and pee.. on the day with boxer, i never buttoned any, so just open it and pull it out then pee..

I usually undo my pants and flop it out my underwear. Using the fly can be difficult depending upon the jeans etc. But, then with my low rise jeans, I sometimes just reach in under my waistband and pull it out over top without undoing anything. It all depends.
